If you have scheduled an initial visit at Primary
Naturopathic, we ask that you complete a
welcome packet prior to your first appointment.
The packet includes a series of forms to apprise the
doctor of your personal and medical history and a
diet diary with 3 days worth of data on your
typical eating habits.
The forms provided below must be returned at least
2 business days in advance of your appointment. Please remember to get started on your diet diary early as it can require almost a week to complete.
It is also helpful to include any pertinent medical records such as recent labwork if possible.
